LAST DISCUSSION ON CRAVEN COUNTY

The Church, Education and Business Organizations
should join together in educating the poppulation on
healthy ways to live to prevent cardiac diseases. The
craven county poppulation should be educated on the
best nutrition to maintain, the importance of
exercising frequently and avoiding smoking and
drinking. This are some of the cardiac programs that
can prevent and reduce cardiac morbidity and mortality
in addition to frequent screening for cardiac
diseases.

The National Heart Association, American Heart
Association in collaboration with the Craven County
Health Department can prepare the department's health
care professionals on the best way to approach the
craven poppulation to educate and motivate them to
lead a healthy lifestyle. This can prevent, reduce
cardiac morbidity and mortality.

Lower income families are elegible for the Medicaid
program. This can help support cardiac programs for
the lower income people. The health department's
health educators can educate the craven county
poppulation on the best nutrition, importance of
exercising and staying away from smoking and drinking.
The county Health Department can also offer frequent
screening services to the community members.

The Health Department in collaboration with the
education department should implement programs for
Disaster Relief. The Households, businesses, and
schools should have contigent programs incase of a
hurricane. Indorder for the department to provide
county wide services during a hurricane it should
educate students on basics of disaster relief this can
increase the number of volunteers incase of an
emergency.

The county can also make an effort to start emergency
stations throughout the county with trained Emergency
Medical Technicians (EMT) and Disaster Management
Professionals in additional to volunteers. Efficient
coodination between the stations and professonals can
help in providing effective services.

Pollution from Industry and Vehicle emissions has
reduced the quality of the air. In addition to this,
Craven County is an agricultural county. Pollens from
the plants can be triggers of asthma, making the
number of asthma patients increase. The Environmental
Protection Agency (EPA) and the Asthma Coalition would
work on improviing the quality of air at home and at
work.

Respiration Therapist and Ear Nose and Throat (ENT)
specialists offer quality services to the asthma
patients. Family physicians can serve dual roles and
give both general and specialist services.

Teachers and parents should educate children on the
importance of preserving air quality. This children
will be future leaders and they will grow up knowing
the importance of quality air. The teachers and
parents should educate the children on the triggers of
asthma and how to live in an environment that makes
one susceptible to asthma.

Providers should provide evidence based practise by
utilizing Information Communication Technology (ICT).
Internet is a source of health care knowledge that can
help the rural providers be at the same level with the
urban providers.

Telehealth and Teleconferencing (Telecommunication)
can be utilized by family physicians and specialist to
give psychiatry services and medical services that
might not need one to travel to the hospital. This can
reduce the cost, accessibility and increase the
quality of health care.

Information Communication Technology (ICT) has
enhanced collaboration. It has made communication
between health care professionals faster and
effective. Also the handling of medical records has
become efficient and timely by efforts being made to
fully emplement the Electronic Health Record (EHR).
Technology has made collobaration be effective and
health care services safer, efficient , timely and
effective.

JULY 18th Reflection VNM

How did the Chat today change your understanding of the cases.

The July 11th Chat session emphasized the importance of having an interoperable health care system. It portrayed how communication can make all the health care professionals work effectively in giving quality services to the patients. Both cases needed the services of the Health Care Department. Health Education can improve the health of the population. Both cases would need the health department to screen the population for possible cases. The mission of Craven County Health Department is:
"… to preserve health than to recover it… to prevent disease than to cure it." -- Dr. George Cheyne, 1725

What did you learn from the cases about your county health department?

The Craven County Health Department has programs conducive for patients with Sexually Transmitted Diseases (STD) and Diseases spread by disease coursing organisms. It has a Health Education Department, Environmental Health Department, WIC/Nutrition Health Department and other departments that can offer quality services to patients and improve the quality of health care in Craven County.

How has your perspective on the role of the health department and collaboration changed as a result of studying the cases?

Information Communication Technology (ICT) has made collaboration/communication easier locally, nationally and internationally. Due to improved means of communication it is easier for the health department to exchange ideas and solve problems in an evidence based manner to improve the quality health care in the county/state/national population.

The Health Department has a very important obligation in the community. It is responsible for controlling the spread of Sexually Transmitted Diseases (STD) and other illnesses. The Environment that inhabits a given population should be conducive for living. This can be made possible by screening community members for communicable illnesses and destroying the habitat of disease coursing organisms.

July 11 Case Studies

CASE 1 BY VNM

(1a) What is the role of the interdisciplinary team members in the delivery of care to the patient

The role of the interdisciplinary team is to ensure the patient receives the best quality of care. By communicating effectively and utilizing the healthcare facilities they can meet the needs of the patient.

(1b) What disciplines have been involved in the case thus far and what other disciplines can be anticipated

The following are the disciplines involved in this case:

Nurse
Physician (MD)
Emergency Transport Staff
Infectious Disease Specialist (MD)
Case Managers
Lab Technicians
Pathologists

(2a) What services could the Simulated Health Department provide to the patient

The Nurse does the assessment, the physician wrights a referral to a specialist after examining the patient, the specialist examines and makes a medical decision based on the patients history and current status.

(2b) What services will need to be provided to the public as a result of this diagnosis

As a result of this case the Department of Health (DOH), Environmental Protection Agency (EPA) and the Department of Agriculture will have to make a decision for craven county to be sprayed with by airplane to reduce the causative agents of Eastern Equine Encephalitis.

This Virus is “susceptible to many disinfectants including 1% sodium hypochlorite, 70%
ethanol, 2% glutaraldehyde and formaldehyde. They can also be destroyed by moist or dry heat, as well as drying” (CDC 2007).

Treatment consists of supportive care. Investigational EEE vaccines may be available for
humans at high risk of infection.

3) What documentation needs are associated with this case

On the subject of documentation of the patient’s health care status the patient may be advised to keep his or her own Personal Health Record. The Health Care Center can utilize an Electronic Health Record System or Paper based Health Care record. It is very essential for the healthcare system to keep a record on the status of the patient at all times.

CASE 2 FROM VNM

(1) What is the composition of health care team necessary to deliver services

(b) What is the identification of resources and service both from the Simulated Health Department and community that can be used to provide care to the patient


The nurse will provide the assessment of the patient. The physician will examine and make a medical decision on the state of the patient. The health care educator should provide helpful information on how the patient should associate with the community members and how to live with the illness.

Since the patient comes from an economically secure family the patient should be able to pay for health care from out of pocket or through his health care insurance.


(c) What is the patient's role in his/her care


The patients should be able to read and understand the informed consent. He should be able to communicate effectively when providing the past family and social history and the history of the present illness.

The patients should be able to read any health care material he receives from the health department on his illness.

(d) What public health risks does this case create? Be comprehensive in thinking about the patient as an expansion of the community in this case and services that will be needed for the aggregate.

Doctors are required to report newly diagnosed cases to the state health department and CDC because this is a sexually transmitted disease and can easily be transmitted to the public because of promiscuity.

The community should receive the following from the Health Care Department:

1) education and counseling of persons at risk on ways to avoid STDs through changes in sexual behaviors;

2) identification of asymptomatically infected persons and of symptomatic persons unlikely to seek diagnostic and treatment services;

3) effective diagnosis and treatment of infected persons;

4) evaluation, treatment, and counseling of sex partners of persons who are infected with an STD; and

5) preexposure vaccination of persons at risk for vaccine-preventable STD

Both cases need Population oriented health care. The cases can turn into an epidemic or pandemic within a short period. Therefore educating the community and taking preventive measures can help reduce the spread of the diseases.

July 11th Case Studies

1a) It is the job of the interdisciplary team to ensure the best patient care possible. They must seek to heal the patient quickly and keep the patient comfortable in the meantime. They should also get her the best treatment available.

1b) Nurses and Doctors at the regional trauma center at PCMH have been involved as well as those at the Craven County Health Clinic. The staff at 911 has also helped. In addition, lab specialists have done work on her case. Other anticipated disciplines include the local health department, environmental specialist and possibly the press if the public should be warned of this infection.

2a) Some services that should be provided are bedside assistance, proper medical care, proper assistance with everyday activities, and financial advice for paying for the care.

2b) The public should be made aware and warned of this condition. They should be told the proper precautions to take to stay clear of the condition. They should also be tested for it. If there is a sort of immunization, of course, this should be administered.

3) There needs to be proper documentation from the Craven County Health Clinic, PCMH trauma center, lab results, and the patients history.



1a) The health care team needs to include doctors, nurses, lab specialists, and financial specialists.

1b) Craven Central and the local health department can both be used to provide care to the patient.

1c) The patients role is to practice safe sex and living habits. He should also inform all of his past partners of his condions. He needs to take medication as prescribed and follow through with the suggested root canal.

1d) This case creates a problem in that the patient is capable and probably already has spread this STD. In addition, his infected partners may spread it as well. If left unattended, the condition could contaminate a large number of the population. Therefore, it is essential to treat not only the primary patient but the others that may be in danger.

We communicate mostly by email. We also chat via blackboard and make postings on our discussion board.

How can you transfer these teamwork skills benefit you in the future?

These skills will ensure future success and productivity when working with teams in the workforce. I will be better experienced and more educated on what methods are and are not effective. This should reduce conflict and increase efficiency.

Why is teamwork important to health care?

Teamwork is important to health care because it increase efficiency. It is the most productive means of delivering health care. There are a limited number of health care professionals compared to the growing patient population. Therefore, working in teams is essential.

Communication and collaboration through information technology has made out team effort efficient, effective and timely.

How are you communicating with your team member?

Information and Communication Technology (ICT) is making my class successful. Internet connection and other telecommunication efforts have made communication with my team member possible.

How can you transfer these teamwork skills benefit you in the future?

The teamwork skills can be transferred in to the future by investing in Information Technology Network. Of course efforts are being made by the National Health Information Infrastructure (NHII) and the Health Resources and Services Administration (HRSA) to ensure the health care system in the rural area has its technology updated.

Why is teamwork important to health care?

Interoperability of the interdisciplinary health care team can reduce medical errors, increase the quality of the healthcare and make the healthcare services cost effective to the patients.

JUNE 27 REFLECTION VNM

How does the availability or lack of availability of health professionals impact rural health?

Lack of health professionals in the rural area can make accessibility and the quality of the healthcare worse. The mortality rate will be a concern and the care the baby boom population receives will be a concern.

The availability of health professions can resolve this problems.

How do we recruit health professionals to your county?

Priority is given to the grandaunts and residents of the county. County job fairs are carried out in the local education, religious and business centers. Incentives and good salary offers are utilized in recruiting.

What are the barriers and the assets of the community that could be used to retain health professionals in rural areas?

Lack of facilities and equipments for the healthcare system, specialist care and other healthcare professionals can make healthcare in the rural area difficult.

Culture, language and socioeconomic differences can also be a barrier in the county healthcare system.

Incentives and offering acceptable salaries can help in retaining healthcare professionals in the county.
