The Church, Education and Business Organizations
should join together in educating the poppulation on
healthy ways to live to prevent cardiac diseases. The
craven county poppulation should be educated on the
best nutrition to maintain, the importance of
exercising frequently and avoiding smoking and
drinking. This are some of the cardiac programs that
can prevent and reduce cardiac morbidity and mortality
in addition to frequent screening for cardiac
diseases.

The National Heart Association, American Heart
Association in collaboration with the Craven County
Health Department can prepare the department's health
care professionals on the best way to approach the
craven poppulation to educate and motivate them to
lead a healthy lifestyle. This can prevent, reduce
cardiac morbidity and mortality.

Lower income families are elegible for the Medicaid
program. This can help support cardiac programs for
the lower income people. The health department's
health educators can educate the craven county
poppulation on the best nutrition, importance of
exercising and staying away from smoking and drinking.
The county Health Department can also offer frequent
screening services to the community members.

The Health Department in collaboration with the
education department should implement programs for
Disaster Relief. The Households, businesses, and
schools should have contigent programs incase of a
hurricane. Indorder for the department to provide
county wide services during a hurricane it should
educate students on basics of disaster relief this can
increase the number of volunteers incase of an
emergency.

The county can also make an effort to start emergency
stations throughout the county with trained Emergency
Medical Technicians (EMT) and Disaster Management
Professionals in additional to volunteers. Efficient
coodination between the stations and professonals can
help in providing effective services.

Pollution from Industry and Vehicle emissions has
reduced the quality of the air. In addition to this,
Craven County is an agricultural county. Pollens from
the plants can be triggers of asthma, making the
number of asthma patients increase. The Environmental
Protection Agency (EPA) and the Asthma Coalition would
work on improviing the quality of air at home and at
work.

Respiration Therapist and Ear Nose and Throat (ENT)
specialists offer quality services to the asthma
patients. Family physicians can serve dual roles and
give both general and specialist services.

Teachers and parents should educate children on the
importance of preserving air quality. This children
will be future leaders and they will grow up knowing
the importance of quality air. The teachers and
parents should educate the children on the triggers of
asthma and how to live in an environment that makes
one susceptible to asthma.

Providers should provide evidence based practise by
utilizing Information Communication Technology (ICT).
Internet is a source of health care knowledge that can
help the rural providers be at the same level with the
urban providers.

Telehealth and Teleconferencing (Telecommunication)
can be utilized by family physicians and specialist to
give psychiatry services and medical services that
might not need one to travel to the hospital. This can
reduce the cost, accessibility and increase the
quality of health care.

Information Communication Technology (ICT) has
enhanced collaboration. It has made communication
between health care professionals faster and
effective. Also the handling of medical records has
become efficient and timely by efforts being made to
fully emplement the Electronic Health Record (EHR).
Technology has made collobaration be effective and
health care services safer, efficient , timely and
effective.

JULY 18th Reflection VNM

How did the Chat today change your understanding of the cases.

The July 11th Chat session emphasized the importance of having an interoperable health care system. It portrayed how communication can make all the health care professionals work effectively in giving quality services to the patients. Both cases needed the services of the Health Care Department. Health Education can improve the health of the population. Both cases would need the health department to screen the population for possible cases. The mission of Craven County Health Department is:
"… to preserve health than to recover it… to prevent disease than to cure it." -- Dr. George Cheyne, 1725

What did you learn from the cases about your county health department?

The Craven County Health Department has programs conducive for patients with Sexually Transmitted Diseases (STD) and Diseases spread by disease coursing organisms. It has a Health Education Department, Environmental Health Department, WIC/Nutrition Health Department and other departments that can offer quality services to patients and improve the quality of health care in Craven County.

How has your perspective on the role of the health department and collaboration changed as a result of studying the cases?

Information Communication Technology (ICT) has made collaboration/communication easier locally, nationally and internationally. Due to improved means of communication it is easier for the health department to exchange ideas and solve problems in an evidence based manner to improve the quality health care in the county/state/national population.

The Health Department has a very important obligation in the community. It is responsible for controlling the spread of Sexually Transmitted Diseases (STD) and other illnesses. The Environment that inhabits a given population should be conducive for living. This can be made possible by screening community members for communicable illnesses and destroying the habitat of disease coursing organisms.

July 11 Case Studies

CASE 1 BY VNM

(1a) What is the role of the interdisciplinary team members in the delivery of care to the patient

The role of the interdisciplinary team is to ensure the patient receives the best quality of care. By communicating effectively and utilizing the healthcare facilities they can meet the needs of the patient.

(1b) What disciplines have been involved in the case thus far and what other disciplines can be anticipated

The following are the disciplines involved in this case:

Lab Technicians
Pathologists

(2a) What services could the Simulated Health Department provide to the patient

The Nurse does the assessment, the physician wrights a referral to a specialist after examining the patient, the specialist examines and makes a medical decision based on the patients history and current status.

(2b) What services will need to be provided to the public as a result of this diagnosis

As a result of this case the Department of Health (DOH), Environmental Protection Agency (EPA) and the Department of Agriculture will have to make a decision for craven county to be sprayed with by airplane to reduce the causative agents of Eastern Equine Encephalitis.

This Virus is “susceptible to many disinfectants including 1% sodium hypochlorite, 70%
ethanol, 2% glutaraldehyde and formaldehyde. They can also be destroyed by moist or dry heat, as well as drying” (CDC 2007).

Treatment consists of supportive care. Investigational EEE vaccines may be available for
humans at high risk of infection.

3) What documentation needs are associated with this case

On the subject of documentation of the patient’s health care status the patient may be advised to keep his or her own Personal Health Record. The Health Care Center can utilize an Electronic Health Record System or Paper based Health Care record. It is very essential for the healthcare system to keep a record on the status of the patient at all times.

(b) What is the identification of resources and service both from the Simulated Health Department and community that can be used to provide care to the patient


The nurse will provide the assessment of the patient. The physician will examine and make a medical decision on the state of the patient. The health care educator should provide helpful information on how the patient should associate with the community members and how to live with the illness.

Since the patient comes from an economically secure family the patient should be able to pay for health care from out of pocket or through his health care insurance.


(c) What is the patient's role in his/her care


The patients should be able to read and understand the informed consent. He should be able to communicate effectively when providing the past family and social history and the history of the present illness.

The patients should be able to read any health care material he receives from the health department on his illness.

(d) What public health risks does this case create? Be comprehensive in thinking about the patient as an expansion of the community in this case and services that will be needed for the aggregate.

Doctors are required to report newly diagnosed cases to the state health department and CDC because this is a sexually transmitted disease and can easily be transmitted to the public because of promiscuity.

The community should receive the following from the Health Care Department:

1) education and counseling of persons at risk on ways to avoid STDs through changes in sexual behaviors;

2) identification of asymptomatically infected persons and of symptomatic persons unlikely to seek diagnostic and treatment services;

3) effective diagnosis and treatment of infected persons;

4) evaluation, treatment, and counseling of sex partners of persons who are infected with an STD; and

5) preexposure vaccination of persons at risk for vaccine-preventable STD

Both cases need Population oriented health care. The cases can turn into an epidemic or pandemic within a short period. Therefore educating the community and taking preventive measures can help reduce the spread of the diseases.

July 11th Case Studies

1a) It is the job of the interdisciplary team to ensure the best patient care possible. They must seek to heal the patient quickly and keep the patient comfortable in the meantime. They should also get her the best treatment available.

1b) Nurses and Doctors at the regional trauma center at PCMH have been involved as well as those at the Craven County Health Clinic. The staff at 911 has also helped. In addition, lab specialists have done work on her case. Other anticipated disciplines include the local health department, environmental specialist and possibly the press if the public should be warned of this infection.

2a) Some services that should be provided are bedside assistance, proper medical care, proper assistance with everyday activities, and financial advice for paying for the care.

2b) The public should be made aware and warned of this condition. They should be told the proper precautions to take to stay clear of the condition. They should also be tested for it. If there is a sort of immunization, of course, this should be administered.

3) There needs to be proper documentation from the Craven County Health Clinic, PCMH trauma center, lab results, and the patients history.



1a) The health care team needs to include doctors, nurses, lab specialists, and financial specialists.

1b) Craven Central and the local health department can both be used to provide care to the patient.

1c) The patients role is to practice safe sex and living habits. He should also inform all of his past partners of his condions. He needs to take medication as prescribed and follow through with the suggested root canal.

1d) This case creates a problem in that the patient is capable and probably already has spread this STD. In addition, his infected partners may spread it as well. If left unattended, the condition could contaminate a large number of the population. Therefore, it is essential to treat not only the primary patient but the others that may be in danger.

Working as a team has been productive and efficient. We communicate via email and blackboard postings. This seems very effective.

We communicate mostly by email. We also chat via blackboard and make postings on our discussion board.

How can you transfer these teamwork skills benefit you in the future?

These skills will ensure future success and productivity when working with teams in the workforce. I will be better experienced and more educated on what methods are and are not effective. This should reduce conflict and increase efficiency.

Why is teamwork important to health care?

Teamwork is important to health care because it increase efficiency. It is the most productive means of delivering health care. There are a limited number of health care professionals compared to the growing patient population. Therefore, working in teams is essential.

How are you communicating with your team member?

Information and Communication Technology (ICT) is making my class successful. Internet connection and other telecommunication efforts have made communication with my team member possible.

How can you transfer these teamwork skills benefit you in the future?

The teamwork skills can be transferred in to the future by investing in Information Technology Network. Of course efforts are being made by the National Health Information Infrastructure (NHII) and the Health Resources and Services Administration (HRSA) to ensure the health care system in the rural area has its technology updated.

Why is teamwork important to health care?

Interoperability of the interdisciplinary health care team can reduce medical errors, increase the quality of the healthcare and make the healthcare services cost effective to the patients.

How does the availability or lack of availability of health professionals impact rural health?

Lack of health professionals in the rural area can make accessibility and the quality of the healthcare worse. The mortality rate will be a concern and the care the baby boom population receives will be a concern.

The availability of health professions can resolve this problems.

How do we recruit health professionals to your county?

Priority is given to the grandaunts and residents of the county. County job fairs are carried out in the local education, religious and business centers. Incentives and good salary offers are utilized in recruiting.

What are the barriers and the assets of the community that could be used to retain health professionals in rural areas?

Lack of facilities and equipments for the healthcare system, specialist care and other healthcare professionals can make healthcare in the rural area difficult.

Culture, language and socioeconomic differences can also be a barrier in the county healthcare system.

Incentives and offering acceptable salaries can help in retaining healthcare professionals in the county.

Session 7

How does the availability or lack of availability of health professionals impact rural health?

This has become particularly clear to me over the past week. The availability of a health professional is very important for rural health in general. It is essential that they be available as much as possible in order to ensure productivity and safety within the community. I had several questions to ask a health profession as part of my assignment from class. She responded fairly quickly saying she was willing to help. However, it has been several days and I have not yet received her responses to my questions. As a result, my learning in this area has been put on hold. It is absolutely vital that health professions be available to their community in order to maintain positive rural health.

How do we recruit health professionals to your county?

The general way of hiring/recruiting health professionals is to look for college graduates. When doing so it is important to research their background, that is, area of study, extra ciricular activities, etc. It is also important not to overlook those that do not have a college degree. Oftentimes, these individuals have enough experience to fullfil the position.

What are the barriers and the assets of the community that could be used to retain health professionals in rural areas?

Some barriers are of course population. The area, being rural, does not have many people living nearby. This makes selection limited. Also, considering the county is rural, a limited amount of people are familiar with health positions available. However, the county does have a well developed health program. And of course there is always room for expansion. They must continue to progress. They should also try to reach out to the public to inform them of the programs they have available.

Session 6 - Week of June 30

What are the challenges of learning online?

I have found that learning online is often challenging due to response time. By this I mean, if one is faced with a question or problem, it is sometimes difficult to get an immediate response back. This may be due to a timing conflict, etc. It is also sometimes difficult to find information that is needed. There is so much information on the web that it can be difficult to pick out what is necessary. However, with the presense of other technology such as telephone, email, fax, etc. learning online is actually very effective.


How have you overcome these challenges to locate information needed for your report?

I have overcome these challenges, or should I say am working on overcoming them in several ways. First of all, I have learned that it is absolutely essential to ask questions when necessary. I have never been one to ask for help, but I have learned that sometimes this is the best alternative in order to be successful. Another lesson I have learned is the importance of planning and organizing. When being presented with a report on a scale such as this one, it is vital to stay organized and on task. Procrastination causes one to work that much harder.


How have you been able to reach out into the community and get to know them using electronic communications?

Here again, the act of asking questions has assisted me a great deal. By making use of electronic means of communication, I have been able to talk to people miles and miles away from me in a matter of minutes.


What have you learned this week that is important to your role as a health professional?

I have learned that no matter what, there is always room to grow. Such growth can be accomplished through effective communication.


Why is what you are learning important to health care?

This sort of learning is important to health care for several reasons. First of all, it expands ones horizons by familiarizing one with information that may be seen in later in life. For instances, I may have to do a report on a community's health care program later in my career. In addition, this sort of learning teaches how to communicate with others and how to reach out to the community. It teaches how to communicate electronically and how useful this sort of communication may be.

What are the challenges of learning online?

With Video Conferencing, Phone over the Internet, live chatting with the instructor and numerous browsers to use in browsing...there is little challenge in learning on line like technology problem i.e. system down time.

How have you overcome these challenges to locate information needed for your report?

Yes. Timing can help in overcoming the system down time. However, i notice slow response to my e-mails from the Craven County Health Department.

How have you been able to reach out into the community and get to know them using electronic communications?

Common situations have led me to utilizing the Internet for assistance from the others with a similar situation. For example, health concerns have made me join the yahoo health chatting forums. Also, preliminary activism efforts to amend the constitution has led me to joining chatting forums.


What have you learned this week that is important to your role as a health professional?

There is a lot that is undone in the Health Care System. First: we need a better system of ensuring everyone in the U.S. has a health insurance. Second: Medical Errors can be reduced if the health care systems implement the elements stated in the IOM report (Crossing the Quality Chasm). Third: EHR can improve the management of Medical Records. Lastly, effective communication/collaboration can improve health care administration.

Why is what you are learning important to health care?

Knowledge is the key to the success of our health care system. The rural health care can be a better system with educated health care professionals. Therefore I am preparing myself to be a health care professional. Everything we are learning in IRHE 3100 is preparing me for the challenges in the rural health care system.

Week of June 13th - Session 5

What have you done thus far in the class?

So far we have completed two online chat sessions, that is, Dr. Greer, Victor, and myself. During the first discussion, we reviewed the course objectives, assignments and posted a brief introduction about ourselves and our past experience in health care. Dr. Greer has questioned us on areas that are important to our studies. In addition, she has guided us through what we should be doing for our group study.

We have been told that we will be doing a study on Craven County's Public Health Care System. Will be designing our own system based on what we believe are the major issues of the community.

Why is this learning important to you as a future health professional?

This sort of learning is important to me as a future health professional because of the different disciplines that it incorporates. It teaches teamwork above all, a vital quality in the health care field. It also teaches the importance of research regarding different projects.

Why is it important to other health professionals that you may work with in patient care?

Of course this is important to the other professionals in the work force in order to ensure effective communication and productive activity.

How is what you are learning important to the rural health of Eastern NC?

It is important to learn as much as possible about the area that one in living, in this case, Eastern NC. By being aware of the issues and concerns of the community one may be more prepared to better the community as best as possible.

Week of June 6th - Session 4

What have you learned about how rural health is defined thus far?

Little did I know, the term "rural" actually has a very specific definition regarding population density. There are 2,500 people living in a certain area according to the federal government's definition. The ratio of persons per square mile is very important.

What have you learned about the public health system of NC?

North Carolina has a variety of rural and urban areas based on population density. Public health is needed for prevention and maintenance of the state's general health. Vaccination, health education, child and maternal health, and immunization programs help to accomplish this.

What have you learned about the county you have researched?

One very important concept is that cancer is the leading cause of death in Craven County. This is certainly an issue that will need to be addressed. It is also clear the Craven County is rural and slightly underdeveloped. They lack a lot of the health care means needed to maintain appropriate health status.

Why is working as a team important to your career in HIM?

Working as a team is essential in order to accomplish what needs to be accomplished efficiently and effectively. When people work as a team, they are able to complete task more quickly. The quality of work put out by teams in general is "better". Working as a team will assure quality and progress.

What have you learned through this first exercise that will help in you in your role of HIM?

I have learned the importance of addressing only the most important issues at hand. In any case, numerous problems may be present. It is practically impossible to fix all of these problems. Therefore, it is essential to focus on the gravest of topics and work on them in depth. I have also learned that it is vital to research any topic before diving into it. By being more familiar with a topic, one can better work on the issues found.

How was this exercise related to the role of HIM as a part of an interdisciplinary team (if you were an actual member of the Health Department)?

It shows how important it is to have so-called "partners" when working on a project. For instance, in work ing with the issue of smoking cessation in Craven County, it is vital to have supporting partners such as the education system, health care facilities, restaurants, etc. By getting others involved, there is more support for the project and, in turn, a better chance of success.

WEEK OF JUNE 13TH VICTOR MOTURI

What we have done thus far in class.

Reviewed Course Objectives. Posted an introduction of ourselves on Bb. Of special interest in the introduction was the type of involvement we have had with community health care agencies, especially a health department.

We held a chat session on course objectives, course assignments and timeliness.The community assessment as a framework for interdisciplinary rural health care planning was presented in discussion board. We learned about Craven County Health Care System. The statistics indicated the death rate from various diseases and the number of health care professionals available in the county.

Designing a Public Health System for Craven County is possible with the introduction we received during our June 6 chat session. The Public Health System should be based on the health care needs of the county residents. The PH should be designed with the the State regulations and other affiliated organizations and associations' policies to PH.

Why is this learning important to you as a future health professional:

The knowledge from learning about rural health care gives me the big picture of the future of the health care globally. U.S. can serve as an initial template of the best health care system that can serve community members efficiently while offering quality health care. This is just but an initial step to globalizing the health care system. The old adage says charity begins at home.

The project introduced to us the basics of implementing a working health care system. From the administration to implementing effective Information Communication Technology Infrastructure. This defines the roles of the Health care professional that i am learning to be.

Why is it important to other health professionals that you may work with in patient care?

Learning about Rural Health care makes collaboration easier within the work place. It gives the health professionals a common perspective about the rural health care.

How is what you are learning important to the rural health of Eastern NC?

IRHE 3100 exposes students to the Health Care statistics in the NC. The statistics inform the future health care professional on the available resources in Eastern NC and what should be done to improve health care in Eastern NC.

Rural Health Context: Week of May 30th

How does the federal government define rural health?

"Territory, population, and housing units not classified asurban constitute 'rural'." In addition, according to the Census Bureau, urban is defined as "comprising all territory, population, and housing units inurbanized areas and in places of 2,500 or more persons outsideurbanized areas." These definitions differ slightly from my personal definitions. While I was aware that the term "rural" meant any place that was not "urban", I did not know the actual statistics that make urban what it is and rural what it is.

Role of HIM, Public Health Nursing, and other disciplines

The job of an HIM professional is to maintain health care records. This includes vital records and health statistics. Public Health Nursing is a unique discipline that seeks to promote and protect the health of populations using knowledge from nursing, social and public health sciences. They must follow theses principles:

-The client or “unit of care” is the population.
-The primary obligation is to achieve the greatest good for the greatest number of people or the population as a whole.
-The processes used by public health nurses including working with the client(s) as an equal partner.
-Primary prevention is the priority in selecting appropriate activities.
-The focus is on selecting strategies that create healthy environmental, social and economic conditions in which populations may thrive.
-There is an obligation to actively identify and reach out to all who might benefit from a specific activity or service.
- Optimal use of available resources to assure the best overall improvement in the health of the populations is a key element of the practice.
-Collaboration with a variety of other professions, populations, organizations and other stakeholder groups is the most effective way to promote and protect the health of the people.

Mission Statement of Public Health Care Systems in NC:

A wide range of NC public health programs and services on both state and local levels work together to protect and improve the health of the people who live and work in NC state agencies.

Summary of House Bill 718: Suicide Prevention Program

The bill intends to provide the appropriate funding to a program that promotes the awareness of youth suicide.

Section 1: This section lays the guidelines for an advisory committee for the program.
Section 2: This section summarizes the funds/budgeting for the program.
Section 3: This act becomes effective July 1, 2007.

Summary of House Bill 1088: Cancer Prevention and Treatment act 2007

This act intends to provide appropriate funding for the treatment and prevention of cancer.

Section 1: The section merely establishes the act's title as the Cancer Prevention and Treatment Act of 2007.
Section 2: This section defines how much money will be used toward the N.C. tobacco Quit line
Section 3: This section defines how much money will be used for uninsured cancer patients who do not qualify for other publicly funded health coverage.
Section 4: This section defines what funds will be allocated for the NC Brest Cancer and Cervical Cancer Program.
Section 5: This section defines how much money will be used as a State match to draw down funds from the Legacy Foundatin for public education programs aimed at cancer prevention and smoking cessation.
Section 6: This section states that the employer shall meet the standards for related health coverage
Section 7: Sections 1-4 of the act become effective July 1, 2007. The remaind3er of the act is effective when it becomes law.


Summary of House Bill 976: Public Vehicular Areas Defined

This acts claifies the definition of public vehicular area in motor vehicular law. It also gives emergency vehicles access to gated comunities.

Section 1: This section simply defines related definitions and phrases.
Section 2: Part 10 of Article 3 of Chapter 20 of the General Statutes is amended by adding a new section to read:
"§ 20‑158.3. Emergency entry to controlled access roads.
Any person, association, or other legal entity having responsibility for a controlled access system on a road that is a public vehicular area shall provide a means of immediate access to all emergency service vehicles, which shall include law enforcement, fire, rescue, ambulance, and first responder vehicles."
Section 3: The act becomes effective December 1, 2007

4) Why is working as a team important to your career in HIM?

The IOM Healthcare goals i.e. Safety, Quality, effectiveness, efficiency… can be accomplished 1st by proper maintenance of medical records and reimbursement system.
This can be effective in the HIM department by staff members working as a team towards the same goals named above.

5) What have you learned through this first exercise that will help you in your role of HIM?

Continuing Education will reinforce my progress in the HIM profession. Striving for excellence and achieving the necessary experience also prepares one to be a better HIM professional.


6) How was this exercise related to the role of HIM as a part of an interdisciplinary team (if you were an actual member of the Health Department)?

The Healthcare system will not be effective / efficient without proper maintenance of the Medical Records. Proper maintenance of the medical records enables physicians to give quality health care. In addition to this, HIM professionals take responsibility of ensuring that the Health Care Providers are compliant with State, local and accreditation agencies.

AN ACT TO PROVIDE ACCESS TO INFORMATION FOR PUBLIC HEALTH PURPOSES IN A MANNER THAT IS CONSISTENT WITH THE HEALTH INFORMATION PORTABILITY AND ACCOUNTABILITY ACT (HIPAA) MEDICAL PRIVACY RULE.

"§ 130A‑15. Access to information.

(a) Health care providers and persons in charge of health care facilities or laboratories shall, upon request and proper identification, permit the State Health Director to examine, review, and obtain a copy of records containing privileged medical information or information protected under the Health Information Portability and Accountability Act (HIPAA) medical privacy rule, 45 C.F.R. Parts 160 and 164, that the State Health Director deems are necessary to prevent, control, or investigate a disease or health hazard that may present a clear danger to the public health.

(b) Confidential or protected health information received by the State Health Director pursuant to this section shall be confidential and shall not be released, except when the release is made pursuant to any other provision of law, to another federal, state, or local public health agency for the purpose of preventing or controlling a disease or public health hazard, or to a court or law enforcement official or law enforcement officer for the purpose of enforcing the provisions of this Chapter or for the purpose of investigating a disease or public health hazard.

(c) A person who permits examination, review, or copying of records, or who provides copies of the records pursuant to subsection (a) of this section is immune from any civil or criminal liability that might otherwise be incurred or imposed."

SECTION 2. G.S. 130A‑5(2) reads as rewritten:

"(2) To investigate the causes of epidemics and of infectious, communicable and other diseases affecting the public health in order to control and prevent these diseases; to provide, under the rules of the Commission, for the prevention, detection, reporting and control of communicable, infectious or any other diseases or health hazards considered harmful to the public health;

SECTION 3. This act is effective when it becomes law.

AN ACT TO APPROPRIATE FUNDS TO THE Department of Health and Human Services, Division of Public Health, FOR EFFORTS TO raise awareness about HEALTH DISPARITIES AMONG NORTH CAROLINIANS OF DIFFERENT RACIAL

SECTION 1. This act shall be known as the Hall‑Allen‑Hunter‑Holloman‑Lucas‑Martin Health Disparities Reduction Act of 2007.

SECTION 2. Of the funds appropriated in the Current Operations and Capital Improvement Appropriations Act of 2007 to the Department of Health and Human Services, Division of Public Health, for the Community‑Focused Elimination of Health Disparities Initiative, the sum of five hundred thousand dollars ($500,000) in each fiscal year shall be used for concerted efforts to address health disparities among African-American and other minority populations in North Carolina by:

(1) Instituting a pilot program for nurse practitioners to travel throughout the State to provide routine health care at community centers, high schools, and churches.

(2) Providing enhanced education and outreach to minority populations on the prevention, diagnosis, and treatment of heart disease, breast cancer, diabetes, obesity, and HIV infection.

(3) Addressing cultural and communication barriers to quality care by improving interpersonal processes between clinicians and patients.

SECTION 3. This act becomes effective July 1, 2007.

AN ACT TO REQUIRE equity IN HEALTH INSURANCE COVERAGE FOR MENTAL ILLNESS

§ 58‑51‑55. No discrimination against the mentally ill and chemically dependent.dependent individuals.

(b) Coverage of Physical Illness. – No insurance company licensed in this State under this Chapter shall, solely because an individual to be insured has or had a mental illness or chemical dependency:

(1) Refuse to issue or deliver to that individual any policy that affords benefits or coverages for any medical treatment or service for physical illness or injury;

(2) Have a higher premium rate or charge for physical illness or injury coverages or benefits for that individual; or

(3) Reduce physical illness or injury coverages or benefits for that individual.

§ 58‑3‑220. Mental illness benefits coverage.

(a) Mental Health Equity Requirement. – An insurer shall provide in each group health benefit plan benefits for the necessary care and treatment of mental illness that are no less favorable than benefits for physical illness generally. Benefits for treatment of mental illness shall be subject to the same limits as benefits for physical illness generally. For purposes of this subsection, 'limits' includes durational limits, deductibles, coinsurance factors, co‑payments, maximum out‑of‑pocket limits, annual and lifetime dollar limits, and any other dollar limits or fees for covered services.

(b) Weighted Average. – If a health benefit plan contains annual limits, lifetime limits, co‑payments, deductibles, or coinsurance only on selected physical illness and injury benefits, and these benefits do not represent substantially all of the physical illness and injury benefits under the health benefit plan, then the insurer may impose limits on the mental health benefits based on a weighted average of the respective annual, lifetime, co‑payment, deductible, or coinsurance limits on the selected physical illness and injury benefits. The weighted average shall be calculated in accordance with rules adopted by the Commissioner.

§ 58‑65‑90. No discrimination against the mentally ill and chemically dependent.dependent individuals.

(b) Coverage of Physical Illness. – No service corporation governed by this Chapter shall, solely because an individual to be insured has or had a mental illness or chemical dependency:

(1) Refuse to issue or deliver to that individual any individual or group subscriber contract in this State that affords benefits or coverage for medical treatment or service for physical illness or injury;

(2) Have a higher premium rate or charge for physical illness or injury coverages or benefits for that individual; or

(3) Reduce physical illness or injury coverages or benefits for that individual.

"§ 58‑67‑75. No discrimination against the mentally ill and chemically dependent.dependent individuals.

b) Coverage of Physical Illness. – No health maintenance organization governed by this Chapter shall, solely because an individual has or had a mental illness or chemical dependency:

(1) Refuse to enroll that individual in any health care plan covering physical illness or injury;

(2) Have a higher premium rate or charge for physical illness or injury coverages or benefits for that individual; or

(3) Reduce physical illness or injury coverages or benefits for that individual.

SECTION 6. This act becomes effective January 1, 2008, and applies to health benefit plans that are delivered, issued for delivery, or renewed on and after that date. For purposes of this act, renewal of a health benefit policy, contract, or plan is presumed to occur on each anniversary of the date on which coverage was first effective on the person or persons covered by the health benefit plan.

The Principles that guide the Registered Nurses in NC define their main roles.
The following principles were adopted from the NC public health website:

(1) The client or “unit of care” is the population.
(2) The primary obligation is to achieve the greatest good for the greatest number of people or the population as a whole.

Rural Health 2007

Define Rural Health:

The Federal Government Definition:

Territory, population and housing units not classified as urban constitute.

In the 100% data products, "rural" is divided into "places of less than 2,500" people and "not-in-places. " The "not-in-places" category comprises "rural" outside incorporated and census designated places and the rural portions of extended cities.

In the sample data products, rural population and housing units are subdivided into "rural farm" and "rural non-farm."

"Rural farm" comprises all rural household, and housing units on farms.

"Rural non-farm" comprises the remaining rural.

Brittony (IRHE 3100 student) defined rural health as a small town probably with limited facilities and resources. My definition referred to its poor infrastructure like the country roads and the long distance shopping centers. Neither of this was close to the definition Dr. Greer gave as defined by the government relative to the number of persons per land mass or distribution density.
